Visualization of pulmonary vein–left atrium lesions using delayed-enhancement magnetic resonance imaging after cryothermal balloon catheter ablation: A case report

چکیده
Introduction Electrical pulmonary vein (PV) isolation is an effective technique that is used to treat patients with paroxysmal atrial fibrillation (AF). In recent years, PV isolation by cryothermal balloon (CB) ablation has been increasingly adopted by electrophysiology laboratories around the world. This technology is associated with more reproducible results and reduced procedural times and may be less influenced by operator dexterity than the traditional pointby-point approach with radiofrequency catheter ablation. Although it may offer the invaluable advantage of achieving PV isolation with a single application, little is known about the extension of the PV–left atrium (LA) lesion after the procedure in clinical practice. Although delayedenhancement magnetic resonance imaging (DE-MRI) has been reported as a noninvasive tool for visualizing lesions after radiofrequency or CB ablation, the relationship between the CB ablation procedure and lesions on DE-MRI results has not yet been assessed in detail.

Atrial fibrillation (AFib) is the most common heart arrhythmia. In certain situations, it can result in life-threatening complications such as stroke and heart failure. For paroxsysmal AFib, pulmonary vein isolation (PVI) by catheter ablation is the recommended choice of treatment if drug therapy fails. During minimally invasive procedures, electrically active tissue around the pulmonary veins ...
متن کاملEccentric scar formation around a pulmonary vein after cryo-balloon ablation in a patient with atrial fibrillation: A case report
The impact of a cryoballoon ablation is reported to be similar to that of a radiofrequency (RF) ablation in patients with atrial fibrillation. Delayed enhancement magnetic resonance imaging (DE-MRI) could visualize the scar region induced by the cryoballoon ablation as well as RF ablation. Cryoballoon ablation could induce extensive scar lesions around the PVs. However, the distribution of the ...

Pre-procedure X-ray computed tomography (CT) and magnetic resonance imaging (MRI) angiography are commonly used to delineate the complex and variable relationship of the left atrium, pulmonary veins, and surrounding structures. 3D CT and MR angiography are routinely incorporated into electroanatomic mapping systems to guide ablation lesion placement in the context of patient specific anatomy. AIMS Cryothermal energy (CTE) ablation via a balloon catheter (Arctic Front, Cryocath) represents a novel technology for pulmonary vein isolation (PVI). However, balloon-based PVI approaches are associated with phrenic nerve palsy (PNP). We investigated whether 'single big cryoballoon'-deployed CTE lesions can (i) achieve acute electrical PVI without left atrium (LA) imaging and (ii) avoid PNP ...
